I enclose this completed form with a $50 nonrefundable certificate program fee and three stapled application sets, which include my résumé, letter of application, and, for international students, proof of satisfactory TOEFL score for English proficiency. (It is the applicant's responsibility to make sure all required materials are submitted with the application. All applicants are evaluated on the basis of materials submitted.)
