*   In addition to instructional fees, there is a services and activities fee and a technology fee (not charged in Summer Quarter). The amount of these fees is determined by the total number of credits taken each quarter. During our transition to a new online system, the services and activities fee ($40–$140) will not be calculated in the online registration form. This fee will be added to the total instructional fees and your credit card charged accordingly when your registration is processed.

Registration Procedures and Information

UW Educational Outreach (UWEO) is the contact for students registering for courses in the Online Master of Library and Information Science Degree Program. At the time of registration, UWEO will collect fees for coursework and UW services such as registration, services and activities, and technology resources. To register, contact UWEO (see How to Register below).

The Autumn Quarter 2008 cost per graduate credit is $515. Students also pay the services and activities fee, a $35 registration fee each quarter, and, with the exception of Summer Quarter, a technology fee. Below is a brief description of other relevant information and services:

• Payment, or a letter authorizing payment by a third party, must be received at the time of registration. Fees are paid quarter-by-quarter. To avoid late fees, payment must be received prior to the first day of the quarter. Students are not registered without payment.
  
  
• The payment amount must include the total instructional fees, technology fee, services and activities fee, and a $35 nonrefundable, quarterly registration fee.
  
  
• Financial Aid: Financial aid may be available to Online Master of Library and Information Science participants who have filed the Free Application for Federal Student Aid (FAFSA). For more information on eligibility, contact the Office of Student Financial Aid at 206-543-6101. Disbursement of financial aid funds from the University Student Accounts office to UW Educational Outreach is not automated. In order to facilitate the registration process, students with approved or pending financial aid through the Office of Student Financial Aid must sign and submit a UWEO Payment Agreement Form with their registration by fax, mail, or in person. (The form is a PDF file; viewing it requires Adobe Reader.) Upon receipt, you will be enrolled and invoiced for the total fees. Payment of the invoice is due one week after the funds from financial aid are released.
  
  
• Third-Party Payment: For many working professionals, employers function as third-party payers. If your instructional fees are being paid by your employer, a separate document (purchase order or letter of authorization to bill from the employer) must be presented at the time of registration.
  
  
• Student ID Card: The University of Washington issues each student a permanent ID card. This will be issued in-person only at the Online Master of Library and Information Science orientation. You will need to present photo identification to claim your card. A quarterly validation sticker, to be affixed to the ID card, will be mailed to students shortly after they register each quarter.
  
  
• Quarterly U-PASS: A quarterly U-PASS (to be affixed to the ID card included with the quarterly Registration Confirmation and Enrollment Validation) will be mailed to you shortly after you register each quarter. (Note: you will be billed for the U-PASS, even if you are not able to use it, unless you return the U-PASS by the posted deadline.) Important instructions regarding the U-PASS, its use, its return and the payment deadlines will be included.
  
  
• Student Accident and Sickness Insurance Plan: If you are enrolling in the optional Student Accident and Sickness Insurance Plan, you need to do so by the seventh calendar day of the quarter through MyUW. You will be billed separately for this. Insurance fees and account balances are also available on MyUW. You will find more information about this plan on the UW Web site.
  
  
• Payment for the Student Accident and Sickness Insurance Plan and other optional fees will be billed separately through the University fiscal services with payment due on the third Friday of the quarter.
  
  
• Parking: Parking permits, which are valid after 4 p.m. on weeknights and until noon on Saturdays, can be purchased at UW Parking Services, 3901 University Way NE, Seattle (cash or check only). You will find additional parking information, including campus maps, on the UW Web site.
  
  
• The cost of textbooks is extra.
  
  
• The University's tuition exemption policy does not apply to this program.
  
  
• Withdrawals and Course Changes: For information about withdrawals and changes, please see the Academic Calendar on the UW Web site.

Payment Policy:

Payment (or, for students receiving financial aid, the UWEO Payment Agreement) is due at the time of registration. See payment deadlines below. Students receiving financial aid through the UW Student Financial Aid Office must pay in full within one week after the financial aid has been disbursed.

Refund Policy:

You must send a written request to withdraw by the deadlines indicated below.

Payment Policy: Payment (or, for students receiving financial aid, the UWEO Payment Agreement) is due at the time of registration. There is a $25 late fee beginning the first day of the quarter, September 24, 2008. Students receiving financial aid must pay invoice in full within one week after the financial aid has been disbursed. There is a $75 petition fee beginning October 8, 2008. Refund Policy: To receive a 100% refund, minus the $35 registration fee, you must send a written request to withdraw by September 30, 2008. To receive a 50% refund, the written request must be received by October 23, 2008.
